About the Job

Essential Duties & Responsibilities (included but not limited to)

· Perform physical examinations of patients to evaluate health and diagnose illnesses, act as a portal of entry into our clinic, provide prevention education, establish long-term relationships within the community

· Order and perform diagnostic tests, such as ultrasound (in office if trained), x-rays (sent to local center), and laboratory tests

· Perform face-to-face and telemedicine treatments based off need and expansion of clinic offerings (ie. ability to work from home on certain days is a possibility instead of coming into the office)

· Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary.

· Act as a team-member for discussion of patient management and response to care, changing care based off patient need

· Document medical information of patients and maintain electronic medical records

· Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors

· Request consultation or referral with other health care providers, such as physicians and clinics

· Facilitates counseling interventions, psycho education, illness self-management, and family/stakeholder involvement.

· Documents patient’s progress in the clinical record including admission order, progress notes, and discharge recommendation.

· Completes medication forms according to established policy. Completes medication education and consent when possible.

· Knowledge of crisis planning and intervention; assists with patient petition, in-office crisis, and after-hours crisis needs when appropriate

 

Qualifications:

· Graduate degree from an accredited nursing or medical school, such as a Master of Science in nursing or Doctor of Medicine

· Board certified as an Advanced Nurse Practitioner in the specialty area of family medicine Hold an active, unrestricted state Nurse Practitioner license, as well as DEA and NPI number

· Have a current provider card in Basic Life Support (BLS)
