Reading Hospital seeks Certified Registered Nurse Practitioner within Palliative Medicine department. Will provide professional nursing care within an assigned unit according to established procedures, protocols and regulations. Provides comprehensive care to patients with life-limited illnesses. Works closely with team members (physician, nurse, social work and chaplain) to provide relief from physical, psychosocial, and spiritual suffering. Provides clinical care to assigned panel of patients on the inpatient service. The practitioner follows Pennsylvania Nurse Practitioner scope of practice guidelines while integrating quality improvement principles throughout the continuum of care. Responsibilities include: Goals of care conversations with patients and families – helps patients and families determine the next step in a patient’s clinical care; Collaboration with primary team (i.e. hospitalist), consulting services (i.e., cardiology, critical care) and other members of the care team (i.e., social work, chaplain, physical/occupational therapy); Symptom management – pain, nausea, shortness of breath; Call coverage – Works with physicians and Nurse Practitioners on the team to split calls evenly for nights, weekends, and holidays. While “on call”, answers pages, authorizes refills, does new patient consults, and provides ongoing follow-up care; Provides ongoing emotional support to patients/families; Advance Care Planning – Assists patient/families with completion of an Advance Directive and/or POLST form; will assist in hospice call coverage.
MINIMUM REQUIREMENTS: Master of Science degree in Nursing from an accredited school of Nursing; Certificate in Palliative Care Nursing, Must have valid PA Registered Nurse license and PA Certified Registered Nurse Practitioner license; current BLS certification.
